Working Paper Requests
Please follow the process below to obtain a working card:
Required documents:
* Completed Working Paper Application (see below)
* Completed Physical Fitness Certification form (see below)
Please email the completed documents to chsncounselingcenter@ccsd.
Please allow three school days for processing time.
Working cards will be mailed to the address we have on file.
If you prefer to pick up your working card from the School Counseling Center, please include this information in your email. We will communicate via email to arrange a pick up date and time.
**Please note that paper applications are also available in the School Counseling Center.
